Ambulatory Care Coordination

CareCoordinationOur Ambulatory Care Coordination department supports over 300 providers in coordinating care for their patients. They are dedicated to bridging care gaps between inpatient and outpatient care and providing coordinated, efficient care within the patient centered framework.

Our Ambulatory Care Coordinators are tasked with focusing on four specific areas of work: patient care advocacy, hospital discharge transition and follow-up, preventative services, and disease management. In the areas of preventive services and disease management, each care coordinator performs outreach and follow-up activities according to protocols established by our Disease Management Committee.
